**هدف از فعالیتهای بهزیستی در شهرستان گرمه: تامین کمک هزینه خرید دستگاه غربالگری بینایی**
در پی اقدامات 단식을aka بهزیستی در Province شمالی خراسان، شرکت‌ها و مؤسسات فعال در زمینه بهزیستی شهرستان گرمه اطلاع‌رسانی شده‌اند که بتوانند از وام خرید دستگاه غربالگری بینایی برای ارائه خدمات به افراد در معرض خطر زوال بینایی استفاده کنند. در این راستا، کلیه مراکز، موسسات و مجتمع‌های فعال در سطح شهرستان گرمه دعوت به ارائه مدارک لازم برای دریافت کمک هزینه تا تاریخ 28 دی ماه سال 1403 می‌کنند.
**شرایط احراز موسسه**
1. مرکز، موسسه، مجتمع و… باید دارای مجوز معتبر از اداره کل بهزیستی استان خراسان شمالی باشد.
2. در اهداف موسسه و یا مرکز باید اهداف پیشگیری از معلولیتها ذکر شده باشد.
3. داشتن حسن شهرت و سابقه عملکرد مثبت در فعالیتهای موسسه و یا مرکز و مجتمع
4. داشتن فضای فیزیکی مناسب با اجرای برنامه(در دسترس بودن مکان و تجهیزات)
5. داشتن نیروی انسانی باتجربه و آموزش دیده(در صورت عدم داشتن گواهی، پس از معرفی ملزم به شرکت در دوره‌های اعلامی مرتبط از طرف بهزیستی می‌باشد)
6. داشتن شرایط جهت فعال بودن پایگاه دائم در طی سال در موسسه مجری
7. اجرای طرح غربالگری بینایی در سطح شهرها و تمام روستاهای سطح شهرستان
لازم به ذکر است که درخواست‌های ارائه شده متقاضیان پس از تاریخ قید شده ترتیب اثر داده نخواهد شد و انتخاب مرکز یا موسسه پس از انجام مصاحبه و بررسی شرایط احراز صورت خواهد پذیرفت.
